Job Description Summary:

Under the direction of the Sr. Manager, Clinical Risk Management, the Quality and Risk Data Manager shall have responsibility for managing risks related to patient safety, patient satisfaction, and experience, and adhering to Risk Management processes, will insure appropriate and timely reporting, assessment, and management of potential and actual risk situations and will provide support related to external inspections or surveys. Manages patient/provider/prescriber complaints ensuring compliance with grievance protocols and response timelines. Supports a Just Culture by managing and supporting Quality and Safety data obtainment and analysis, including the Quality Key Performance Indicators (KPI), patient satisfaction program, event reporting system, Quality Business Reviews, ad hoc reports for inspections and surveys. Coordinates with various OCH key stakeholders regarding the quality/safety recording system and process and patient satisfaction survey questions, process, and results, providing key feedback and courses of action to the survey vendor(s). Trends quality, safety, and survey response data to identify areas for improvement. Plans and implements training and educational tools for staff using event reporting system. Assesses event management system data to ensure the integrity of data provided by quarterly business reports. Monitors patient satisfaction survey results and identifies opportunities to use the data as a basis for process improvement. Supports access to the patient satisfaction dashboard reports and actions taken as a result.

Job Description:

Job Responsibilities (listed in order of importance and/or time spent)

  • Collaborates and communicates with Sr. Manager, Clinical Risk Management and Director, Quality & Risk Management to identify opportunities for improvement in patient safety and satisfaction and risk mitigation. Collaborates with Director of Quality to oversee the OCH patient satisfaction and event reporting programs.

  • Serves as liaison between various OCH stakeholders, OCH IT department, and patient satisfaction vendor.

  • Leads key stakeholders in identifying specific areas for improvement with patient satisfaction surveys and quality/safety system with a goal to increase event recognition and capture across the organization

  • Creates and analyzes custom reports specific to site, area, therapy, patient age, diagnosis, or nursing service.

  • Presents survey, safety, and quality data and analysis to Quality Director, CQI program manager, clinical, operations, and commercial leaders to share identified opportunities for improvement based on specific findings.

  • Analyzes recommendations for system improvements and presents options, pros, and cons to appropriate leaders/users.

  • Tracks, trends, analyzes event and satisfaction data to identify risks and opportunities for improvement. Identifies and implements opportunities for tracking/trending additional data. Identifies trends in patient events and escalates these to Sr. Risk Manager or other appropriate clinical leaders with recommendations for improvement.

  • Creates and presents reports for operations, clinical and commercial teams, analyzing for accuracy of data. Prepares and presents data for quarterly and annual CQI program, accreditation surveys, and other needs.

  • Develops, implements, and maintains annual training for all users and for managers.

  • Oversees a committee of users to assess functionality and efficiency of the system at least quarterly with the goal of improved ability to track and trend various events. Attends Patient Safety Committee meetings to provide additional insight into the reporting process and develop and provide appropriate reports.

  • Tracks Risk Events to ensure appropriate completion, resolution, follow up and closure.

  • Participates in root cause analysis and corrective action plans as needed, based on patient events or complaints.

  • Assesses potential and actual risk scenarios with patients, caregivers, and staff. Participates in implementation of action plans to mitigate risk and ensure compliance with regulations, accreditation standards, recognized standards of care, and OCH policies.

  • Insures completion of Sedgwick reports when appropriate.

  • Supports legal, compliance, revenue cycle management, operations, Human Resources, and other departments as required for completion of root cause analysis, corrective action plans and risk mitigation.

  • Assists with retrieval of medical records when requested by Legal team.

  • Assists with review and assessment of medical records related to potential or actual litigation.

Basic Education and/or Experience Requirements

  • Bachelor’s degree required.

  • A licensed clinician with a minimum of 2 years of experience in clinical management, patient safety, or risk management activity.

  • A minimum of 2 years of leadership experience with oversight of specific programs such as risk management, employee health, or clinical management programs.

  • Related healthcare quality or data analysis experience.

Basic Qualifications

  • Experienced with OSHA, CDC, ACHC and recognized standards of care for services provided by Option Car Health, and other regulatory and accrediting agency standards.

  • Experience with policies and procedures related to quality improvement and processes.

  • Experience in conducting regular, routine, and focused audits including documenting results of the audits.

  • Experience in patient and customer satisfaction survey programs.

  • Excellent verbal and written communication skills.

  • Excellent organizational skills and ability to multi-task.

  • Proficient with Microsoft Excel, Word, & PowerPoint

  • Ability to read, analyze and interpret general business periodicals, professional journals, technical procedures, or governmental regulation.

  • Ability to write reports, business correspondence, and policy/procedures.

  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the public.

  • Ability to use spreadsheets and databases as well to track and trend data.

Preferred Qualifications & Interests (PQIs)

  • Advanced degree

  • 5 years in a similar role within the home health industry

Due to some state pay transparency laws, below is the minimum pay for the position:

Salary to be determined by the applicant's education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
